Wellness score

The Wellness Score helps you track how you’re feeling over time using a short wellness survey focused on mental, emotional, and physical health. Your responses create a personalized view of your wellbeing that you and your provider can track over time.

Complete your Wellness score check-in

1. In the mobile app, open the Health tab.
2. Select Start check-in.
   
3. Answer the survey questions about mental, emotional, and physical health.
   

4. After completing the survey, you’ll receive an overall Wellness Score.
   
5. Your score is shared with your provider and can be viewed anytime in the Health tab.
   

View your Wellness Score

You can return to the Health tab at any time to view:

• Your current Wellness Score
• Your last updated date
• Previous check-in results

How often you’ll receive check-ins

You’ll be prompted in the app every 4 weeks to complete a new Wellness Score check-in, so you can continue tracking your wellbeing over time.

What providers can view

Providers can view Wellness Scores from the patient profile to help monitor wellbeing over time.

1. Open a patient profile to view the Wellness Score summary at the top of the page.

2. Select any section within the Wellness Score card to view a more detailed breakdown of the patient’s scores and trends over time.
