Understanding the labs service fee
Providers
Patients
Fullscript charges a 3% service fee on the price of specialty tests. We manage the secure storage, shipping, and handling of kits—ensuring your patients receive tests quickly, and that every sample meets the high-quality standards providers expect.
Labs service fee details
When a specialty lab test is added to a plan, a 3% service fee is automatically applied to the cost of that test as a separate line item at checkout.
Example: If an order includes two specialty lab kits priced at $100 each, the 3% service fee would total $6.
Who pays the fee
It depends on who’s paying for the test:
- If the patient is paying for the order, the patient pays for the fee.
- If the provider pays for the order upfront, the fee is charged to the provider. To pay for testing upfront, check I’ll pay for patient’s order when reviewing a plan:

No matter who pays, the fee is limited to specialty labs and is always visible at checkout for full transparency.
Where the fee appears
When a specialty lab kit is included in an order, the service fee is displayed at checkout for patients and providers.
On the checkout screen, this fee will look like:

Specialty labs vs. blood labs
Specialty lab tests involve physical kits stored, shipped, and tracked through Fullscript’s Distribution Centers. The 3% service fee supports this fulfillment process.
Blood lab tests are usually electronic orders, such as those from Quest Diagnostics or Access Labcorp Draws. In some cases, like with Access Medical Labs, the lab ships kits directly to patients. Since Fullscript doesn’t fulfill these kits, the 3% service fee doesn’t apply. To learn more about fees related to blood labs, visit Phlebotomy on Fullscript.
