Completing your lab test with an independent phlebotomist

If your provider orders a specialty lab test for you that requires a blood draw, you may be able to complete it using a local independent phlebotomist. These providers operate separately from Fullscript and may offer mobile or walk-in appointments. This guide walks you through what to expect and how to prepare.

Paying for your testing

If your provider hasn’t already paid for your lab testing, you’ll complete checkout in Fullscript. Your testing cost will be shown clearly during checkout, but phlebotomy with an independent provider is not included in your Fullscript payment —you’ll pay the phlebotomist directly at the time of service.

Locating your requisition form

Once your lab kit arrives, you’ll find your requisition form inside. Bring the physical requisition form with you to your appointment.

Finding an independent phlebotomist

You can easily find independent phlebotomy options through your Fullscript account. During or after checkout, enter your zip code or address to see nearby mobile and walk-in options. Browse the listings to view details like contact information, pricing, and scheduling instructions so you can book directly with the provider that fits your needs.

Before checkout

To browse independent phlebotomy options before placing your lab test order:

  1. On the sample collection step of checkout, select I’ll choose my own sample collection service.

  2. Enter your zip code or address to view a list of local providers.

After checkout

Once you’ve completed checkout, you’ll find this map again after placing your lab order.

  1. In the Schedule your sample collection section, click View options.

  1. Once you’ve found a provider that fits your needs, schedule and pay for the appointment directly with them. Be sure to bring your requisition form and complete test kit with you to your appointment.

Reminder:

Specialty lab kits can’t be drawn at Quest or Labcorp locations.

Scheduling your appointment

Once you choose a provider, follow their instructions to book directly. Be sure to confirm:

  • Whether they accept your lab kit(s).
  • The draw fee and accepted payment methods.
  • Who is responsible for shipping the sample back to the lab — you or the phlebotomist.

Preparing for your test

Your Fullscript account will include any instructions related to fasting or sample collection. Review them before your appointment to avoid delays.

Day of your appointment

Bring the following to your appointment:

  • Your physical requisition form.
  • A valid ID.
  • Your complete lab kit, including all materials and any ice packs.
  • Payment for the phlebotomist (if not already paid).

Shipping your sample

Your sample needs to be shipped to the lab using the prepaid materials included in your kit. Some phlebotomists will handle this for you, while others may ask you to ship it yourself. Ask your phlebotomist who will handle the return shipment, and be prepared to send it yourself if needed.

Viewing your results

After your sample is processed, your results will appear in your Fullscript account. You’ll get an email when your provider has reviewed them and shared their interpretation.
