Filling out your requisition
A requisition form is required for all Jona Advanced/Biomarker tests.
You need to fill out the following fields:
- Patient Information box
- First and last name
- Date of birth
- Sex assigned at birth
- Sample Information box
- Collection date
- Sample barcode ID
- Signature and date
Leave the For Laboratory Use Only section blank.
