Filling out your requisition form
Most Doctor’s Data tests require:
- Sample collection date and time
- Sample type (urine, blood, stool, saliva)
- Additional test-specific context (menstrual status, supplements used, etc.)
Review the table to see which fields you must complete for your particular test:

| Blood panels | -Collection date and time -Fasting status |
| Urine elements | -Collection date and time -Volume -Whether part of a challenge (if applicable) |
| Hair | -Sample type -Hair treatment history -Shampoo used |
| Saliva (hormone panels, CAR) | -Collection date and time -Wake-up time -Menstrual status -Storage confirmation |
| Neuro hormone & neurotransmitter | -Collection date and time -Height -Weight -Menstrual status |
| Cardio/Metabolic | -Collection date and time -Height -Weight -Fasting confirmation |
For tests involving pre/post-loading or challenge protocols, your provider should advise you on what to document. If you’re unsure, contact them directly.
