Requisition form requirements vary by lab company. Use the information below to see what’s needed for your specific lab.

The following labs require you to complete a requisition form at the time of collection. Use the links below for step-by-step instructions for each lab:

Note:
If your provider recommended testing through Quest or Access Labcorp Draw, your requisition is available within your account and you don’t need to fill anything else out.
Kit activation
Certain labs don’t require or include a physical requisition form and instead have a kit activation process. Activation is completed at the warehouse before the kit ships, so no further steps are required after receiving your test kit. This applies to the following labs:
- Genova
- Mosaic Diagnostics
- 3X4 Genetics
Simply collect your sample by following the instructions included with your kit.
