Updating Tax Information
Tax information can be reviewed and updated from the Dispensary Settings within the Tax area as shown below. The details entered here are expected to reflect the information you will submit to the IRS for tax purposes.

Requesting Changes to a Verified Account
Once tax information has been verified for an account, the option to 'Submit and accept terms' will be replaced with the option to 'Submit'. Where this is the case, not all tax changes will be accepted through the form, and you may need to contact our Customer Success team to make the changes with our payment processor, Stripe, directly.
Successful changes will be updated on the Tax page after clicking "Submit", where unsuccessful changes will revert back to displaying the previous information.
Changes that revert after submission will need to be made by contacting Customer Success and may also require a supporting W-9 form.
Further, to make changes to tax information for verified accounts, we will need a completed W-9 form signed by the dispensary owner, with all change requests made directly by the account owner.
Changes requested by other contacts using the dispensary will need to be verified with the account owner through email before changes will be made.
Common Change Requests & W-9 Requirements
You can download a blank version of Form W-9 here. Once complete, send the form as an attachment to our Customer Success team at firstname.lastname@example.org. Guidelines on how best to complete this form when requesting common changes have been provided below.
A completed W-9 is required reflecting the new EIN or SSN for the dispensary, signed by the account owner.
Business Name Changes
A completed W-9 is required reflecting the new name of the business, signed by the account owner.
Business Type Changes
If your business type should be changed from Individual/Sole Proprietor to Business (C Corporation, S Corporation, Partnership, Trust/Estate, or LLC) or vice versa, we will require a completed W-9 with up to date tax information reflected - ie. new business name, address, EIN/SSN, etc.
To submit a Business Type change request, send an email to email@example.com with a summary of the requested changes along with the completed W-9 signed by the account owner.
For businesses, we request you complete the form with the current legal business name and EIN/SSN.
For Individual/Sole Proprietor accounts, the W-9 should be filled with the information of the account owner.
In the Tax section of your Fullscript account, the 'Business Type' can only be toggled when the account has not yet been verified.
Submitted W-9's must be signed by the account owner of the dispensary. The name of the owner/business representative should match the name provided in the personal information section within the Tax page shown below.
Completed W-9's must reflect the information of, and be signed by the account owner.
Changes Not Requiring Form W-9
Financial Address Changes
Changes to your financial address can be changed from the Tax page and submitted directly. You do not need to contact Customer Success or complete a W-9 when making this type of change.
Spelling and DOB entry errors made in the Personal Information section, as shown above, do not require a completed W-9. However, you will need to contact our Customer Success team if you have already submitted your information. In these cases, send an email to firstname.lastname@example.org outlining the mistake and correction(s) necessary.
